Since July of 2016, swinging fire doors in healthcare facilities have been subject to the door safety inspection and testing requirements of NFPA 80, Standard for Fire Doors and Opening Protectives. Many door safety inspection reports include erroneous listings of deficiencies. Some facilities are instructed (and required) to relabel their door frames and doors unnecessarily.
